Application Procedure for International Students

1. You may request an application and forms from Milo or download all of the forms you'll need from the internet.
2. Arrange to have these three recommendations sent to Milo:
   The three people you give your recommendation forms to must not be related to you. The person giving you a recommendation must send their recommendation form directly to Milo Adventist Academy, Admissions Office, P.O. Box 278, Days Creek, OR 97429, USA.
   • Academic Recommendation(34.2 KB) from the student's most recent principal, teacher, guidance counselor, or tutor
   • Spiritual Recommendation(32.9 KB) from the student's current youth leader, spiritual leader, church elder, or pastor
   • Work Recommendation(32.9 KB) from an adult who has observed the student's work ethic, whether the student was paid or a volunteer
3. Fill out, sign, and mail the following forms and documents:
   • Application(34.8 KB)
   • Official copy of translated transcripts
   • Request for Student Records(13.1 KB)This form is needed if the student has already attended school in the US. This form must be signed.
   • Parent Permission(14.7 KB)This form must be signed.
   • International Student Health Information(19.7 KB)This form must be signed.
   • Copy of student's Passport
   • Copy of student's Birth Certificate
   • Proof of Medical Insurance
   • Copy of student's Immunization Records
   • Sponsorship Agreement(13.6 KB)
   • Network Account Agreement(13.2 KB)
   • Riding Release(125.8 KB), if you wish to ride horses

4. When all forms, including recommendations, have been received, the Admissions Director will work with you to set up an appointment for an interview with the Admissions Committee. Please email to arrange for an interview appointment.

5. You will be notified of the acceptance or denial of your application within ten days after your interview.
