The first official transcript after graduation is free. All subsequent transcripts require a fee of $10.00. Make your check payable to Milo Adventist Academy. Please complete the information on the Transcript Request Form Transcript Request Form(52.0 KB) Send your check with your request to: Transcripts Milo Adventist Academy P.O. Box 278 Days Creek, OR 97429 No transcripts can be released for students whose accounts have not been fully paid. Please contact the V.P. for Finance for the status of unpaid accounts. Transcripts sent to the student or faxed are unofficial.
