Application Procedure for New Students

1. You may request an application and forms from Milo or download all of the forms you'll need from the internet.
2. Arrange to have these recommendations sent to Milo:
   • Recommendation(34.2 KB) from your most recent principal/head teacher or guidance counselor
   • Recommendation(32.9 KB) from your current youth leader or pastor
   • Recommendation(32.9 KB) from the employer or supervisor of your most recent job or on-going volunteer project
3. Fill out and mail the following forms:
   • Application(37.2 KB)
   • Request for Student Records(66.6 KB)
   • Parent Permission(56.2 KB)
   • Health Information(15.3 KB)
   • W-4(99.9 KB)
   • I-9(378.8 KB)
   • Network Account Agreement(13.2 KB)
   • Riding Release(362.4 KB), if you wish to ride horses

   When all forms, including recommendations, have been received, the Admissions Director will work with you to set up an appointment for an interview with the Admissions Committee.

4. Bring these items to the interview:
   • Photo ID
   • Immunization Records
   • Birth certificate (original)
   • Proof of health insurance coverage
   • Social Security card (original)
5. You will be notified of the acceptance or denial of your application within ten days after the interview.
